Tuition Reimbursement Potential: NHC’s FQHC status provides a unique opportunity for tuition reimbursement for clinical professionals. This tuition reimbursement program requires application and is not guaranteed. Providers who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. Additionally, there is potential for an additional percentage of repayment for a third year commitment. Neighborhood Health Center is seeking a full time Primary Care Provider (PA). This position will report to the Medical Director and will be based in one site and cover at times in other sites. This position will lead cross-functional care teams. This is a unique opportunity for a provider to develop resources and care for our patients identified as experiencing homelessness and other social determinants of health supported by the community health department. 30-35 hours each week will be dedicated to direct patient care and the remaining hours will support care management, outreach and developing trainings and resources around Health Care for the Homeless services and community health at NHC.

 

The ideal candidate will have a background as a physician assistant with primary care experience, understanding and familiarity working with diverse populations and an education or interest in social determinants of health. This position seeks to provide learning opportunities, understanding and education on race, ethnicity and language, and sexual orientation and gender identity, social determinants of health, trauma informed care, harm reduction, motivational interviewing techniques and skillful outreach and in-reaching and other related topics. The position is designed to last at least one year in the current design and may be reevaluated annually with a potential shift of responsibilities to best meet the needs of patients.

 

All Neighborhood Health Center offices are located in the greater Buffalo, NY region. The Buffalo-Niagara region boasts a great quality of life, with access to tremendous green space, including two Great Lakes and Niagara Falls, world-class museums, professional sports, public concerts and plays, and historic architecture. The area is also incredibly accessible, both in terms of average daily commute time as well having easy access to other major cities (Boston, Chicago, Detroit, New York City and Philadelphia are all about an hour’s flight away while Toronto is only 90 minutes away by car). Responsibilities

•Work as part of the NHC provider staff seeing patients in face to face visits and abide by all the requirements of being part of the NHC medical-dental staff

•Provide high quality primary care to Neighborhood Health Center’s patients of infant, pediatric, adolescent, adult and geriatric groups

•Lead cross-functional patient-centric care teams

•Provide comprehensive patient care and establish a family partnership whenever possible to ensure the needs of the patients are met on an ongoing basis

•Ensure high quality primary care is delivered cost-effectively, consistently

•Provide appropriate specialist referrals and follow-up recommendations when indicated

•Maintain electronic medical records in compliance with Neighborhood Health Center’s standards with high efficiency

•Engage patient, family and cross-functional care teams in sustained health and well-being of the patient

•Partner with Neighborhood Health Center’s interdisciplinary teams and resources

•Participate in learnings provided to develop skill and understanding in community health, health care for the homeless, and special populations (including, but not limited to, refugees, immigrants, veterans, etc.)

•Use the knowledge on these topics to become a trainer, leader and liaison between providers and staff and topics related to health care for the homeless and the community health department of NHC

•Participate in outreach and in-reach efforts especially focusing on patients experiencing homelessness

•Assist in the development and management of advanced care management registry for HCH patients

•Represent Neighborhood Health Center in community outreach activities

•Maintain familiarity with ongoing research and development in relevant fields

•Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions

•Manage a limited, telephone-only call-coverage shared with other clinicians (weekends: every 6-8 weeks; weekdays: every 4 -6 weeks)

 

Requirements

•Physician Assistant, licensed in NYS prior to start date

•Candidates with 4 or more years of experiences will be given preference

•Outstanding “bedside manners”: patient-centric with empathy, excellent active listening and communication skills, engaging and efficient

•Outstanding clinical capabilities and judgment

•Passionate about providing high quality primary health care to low-income, underserved residents and those lacking access to health care

•Focused on collaborative team-based care

•Responsive and comfortable working in a fast-paced, high accountability environment

•Proficient in EHR systems

•Flexible

•Excellent judgment and ability to prioritize resources where they will be most useful

•Comfortable receiving and providing appreciative and corrective feedback

•Engaged in learning and continuous quality improvement

•Outstanding written and verbal communications skills with a culturally diverse patient and staff population

•Proficient in working with in-person or telephonic interpreters
